Grey Bruce Hospice is seeking a full-time (37.5 hours per week) Social Worker to join our team. Reporting to the Hospice Director, the Social Worker is responsible for providing system navigation and person-centered palliative care and grief and bereavement support for residents and their families. Working closely with our highly skilled team of care professionals, the successful candidate will provide direct client care to residents and their families to ensure their psychological, emotional, physical and spiritual care needs are addressed in a professional and compassionate manner.
